Explosion Aboard Sludge Vessel Hunts Point: Unauthorized Hot Work Proves Fatal

An explosion aboard the sludge vessel Hunts Point on the Hudson River near Manhattan, New York, United States, killed the ship’s chief engineer on May 24, 2025. The U.S. National Transportation Safety Board (NTSB) has concluded that unauthorized hot work performed above a cargo of flammable sludge was the cause. Five other crew members sustained minor injuries or were uninjured.

Circumstances

The Hunts Point, a 288.7-foot sludge vessel operated by the New York City Department of Environmental Protection (NYC DEP), departed at 08:18 on May 24, 2025, carrying approximately 40,000 cubic feet of residual sludge. To keep the cargo pumpable, water had to be added through modified hatch covers on the cargo tanks.

The pipe plugs intended for this purpose had rusted and seized. Instead, the chief engineer removed flanges. When bolts sheared off in the process, he reached for a propane heating torch to loosen them — open-flame work performed above a cleaning hatch on cargo tank No. 2, port side. No hot work permit had been issued, and the captain had explicitly warned before the voyage against using a torch near the tanks.

Shortly after this work began, a yellow flame became visible above holes drilled in the hatch cover. At approximately 10:20, an explosion occurred in the tank: methane gas, produced by the decomposing sludge, had ignited and generated a pressure wave inside the cargo tank. The chief engineer was thrown by the blast between the pier and the vessel and was pronounced dead at the scene by the New York City Fire Department. The captain, the second engineer, and an ordinary seaman sustained minor injuries; the mate and the second ordinary seaman were uninjured.

Conclusions

The NTSB determined that the probable cause of the explosion was the performance of unauthorized hot work above a tank containing cargo that produced flammable methane gas, which led to ignition of the gas and the subsequent overpressure buildup in the cargo tank.

The report cites the following contributing factors:

  • the absence of an approved hot work permit;
  • insufficient training of the chief engineer regarding hot work hazards and the risks of sludge gas;
  • disregard of the captain’s explicit warning;
  • rusted and seized pipe plugs, which forced a deviation from standard procedure into a riskier method;
  • the absence of gas testing or a gas-free certification of the tank prior to the hot work;
  • the cleaning hatch being open during the use of an open flame.

Recommendations / lessons

The report does not contain a separate chapter of formal, addressed recommendations. Nevertheless, clear lessons for the industry emerge from the established cause and contributing factors, as well as from the measures NYC DEP, as operator, has already taken:

  • **Hot work permit procedures are not a formality.** Work involving an open flame near tanks containing flammable or decomposing cargo should proceed only under a formal, authorized permit process — including gas testing and, where necessary, a gas-free certification.
  • **Verbal warnings from supervisors must be backed by policy and oversight**, not left as a one-time instruction in the moment.
  • **Deferred maintenance on basic equipment** — such as seized pipe plugs — can push crew members to depart from standard procedures and choose riskier improvisations. Preventive maintenance is therefore also a safety measure.
  • **Training on sludge gas and hot work hazards** should be an explicit part of the instruction given to technical personnel on vessels carrying organic waste or sludge, given the risk of methane formation from cargo decomposition.

Following the accident, NYC DEP issued a memorandum on June 23, 2025, directing personnel to strictly comply with hot work procedures, and announced it would replace the fleet’s hatch covers.
